What We Do
Navigation that turns information into action.
Finding a resource is not the same thing as being able to use it. Eligibility rules, paperwork, transportation, phone access, waitlists, and deadlines can make a simple referral difficult to follow.
A navigation relationship.
NLF is being designed to help close that gap through practical next steps, warm connections, and sustained follow-up.
Listen
Start with what is happening now and what matters most to the person seeking support.
Assess
Identify urgent needs, barriers, documents, deadlines, and the order of next steps.
Navigate
Make options easier to understand and help the participant decide what to pursue first.
Connect
Seek verified resources, warm referrals, and human handoffs when available.
Follow Up
Check whether a connection worked and identify the next step when it did not.
Keep the person at the center.
The planned model begins with the person’s own goals. Navigation would help identify barriers and explain available options, with referrals made through informed, consent-based connections.
A referral alone does not show that a need has been met. The aim is to understand whether the connection worked and help identify another path when it did not.
One life. Connected needs.
Housing, work, school, health, documentation, and community can affect one another. The planned approach considers those connections rather than treating each need in isolation.
